If pain, muscle tightness, or sensitivity keeps limiting how you move, dry needling may be one tool that helps you make progress.
At Congruency Therapy & Wellness, however, we don't begin with the assumption that a tight muscle needs a needle.
We begin by asking why the area is painful or guarded and what needs to change to help you move, perform, and feel better.
Dry needling may become part of that plan.
Dry Needling defined
Dry needling is a physical therapy technique that uses a very thin filament needle to stimulate targeted muscles and tissues without injecting medication.
Depending on your presentation, we may use dry needling to help decrease pain or sensitivity, reduce muscle guarding, or make movement more comfortable.
But we don't view dry needling as a stand-alone solution.
The change it creates is where the next part of treatment begins.
Do I need Dry Needling?
Maybe, but you don't need to decide that before you arrive.
Choosing Congruency for physical therapy does not mean you will automatically receive dry needling.
Your therapist will first evaluate your symptoms, movement, mobility, strength, history, activities, and goals.
If dry needling appears likely to help move treatment forward, we'll explain why.
If manual therapy, exercise, movement retraining, mobility work, or another intervention makes more sense, we'll use that instead.
Dry needling should fit the person.
The person should never have to fit the technique.
What an Appointment Is Like
Your appointment begins with you, not the needle.
We'll talk about what you're experiencing, when it occurs, what you've tried, what activities matter to you, and what you want to get back to doing.
Your therapist will then assess your movement and determine what may be contributing to your symptoms.
If dry needling is appropriate, we'll explain:
· Why we're recommending it
· What area we plan to address
· What you may feel
· What to expect afterward
· What we want to accomplish with the technique
The initial insertion of the thin needle is often minimally noticeable. When a sensitive area is treated, you may experience a brief ache, pressure, cramp, or twitch.
Some muscle soreness afterward can occur.
Most importantly, dry needling usually represents only one portion of your appointment.
What happens next
This is an important part of the Congruency approach.
Suppose dry needling decreases shoulder sensitivity and suddenly reaching overhead is easier.
That's useful.
But we don't want to stop there.
Now we have an opportunity to work on whatever your evaluation showed was limiting you—perhaps shoulder blade control, thoracic mobility, rotator cuff strength, or your ability to tolerate overhead loading.
Or maybe dry needling decreases calf discomfort enough that we can more effectively strengthen the calf and prepare it for running.
The goal isn't simply to make the area feel different.
It's to use that change to help you do something differently.
Your therapist may follow dry needling with manual therapy, neuromuscular retraining, mobility work, exercise, strengthening, or progressive loading.

All of our sessions are 1on1 for 1 hour which give our therapists time to listen, thoroughly assess, and treat your problem.

Comfortable clothing that allows movement and breathing assessment. Preferably limit baggy clothing so assist in our ability to assess your posture/movement.

North Carolina is a full direct access state with no restrictions. This means you do not need a referral for physical therapy unless your insurance specifically requires one for reimbursement purposes.

We are an out of network provider for physical therapy services. We have adopted this model specifically because it gives us more time to assess and treat your specific problem without seeing multiple clients at one time.

Many people barely notice the initial needle insertion. When a sensitive area is treated, you may feel a brief ache, pressure, cramp, or twitch. Temporary soreness afterward is also possible.

There isn't a predetermined number. Your therapist will continually evaluate whether dry needling is producing a useful change and whether that change is helping you progress. If it stops serving a purpose, there's no reason to continue using it simply because it was part of an earlier session.

It's more accurate to think of dry needling as potentially changing pain, sensitivity, muscle response, and movement rather than physically removing a “knot.” If an area repeatedly becomes tight or painful, understanding why it keeps happening is an important part of treatment.
