Part of a larger movement system
Where you hurt matters, but it doesn't always tell us where treatment should begin.
Low back symptoms can be influenced by how your hips and pelvis move, how well your trunk manages load, your mobility, strength, breathing strategies, previous injuries, daily positions, and the demands of your sport or activities.
That's why a low back evaluation at Congruency isn't limited to examining your lumbar spine.
Your therapist may assess your hips, pelvis, rib cage, trunk, lower extremities, posture, balance, strength, and movement patterns. We may also watch you perform the specific movement that gives you trouble.
If deadlifting hurts, we want to see you deadlift. If sitting is the problem, we want to understand how you organize your body while sitting. If your back bothers you during your golf swing, the evaluation needs to consider how you rotate and transfer force throughout your body.
The goal is to understand the relationships contributing to your symptoms rather than simply treating the painful area in isolation.
Your first session at Congruency
Your first appointment begins with a conversation. Your therapist will want to understand what you're experiencing, when it began, what makes it better or worse, previous injuries or treatment, and what your symptoms are preventing you from doing.
Then we evaluate how your body moves. Depending on your presentation, this may include spinal, pelvic, and hip mobility; posture and positional strategies; strength and muscle activation; trunk and core control; functional movement; walking or running mechanics; squatting, bending, or lifting; sport-specific movements; and how different regions of your body work together.
Your treatment begins from what we find during that assessment. Throughout the session, your therapist can treat and reassess rather than separating evaluation from treatment.
A plan individualized to you
Congruency's model allows your physical therapist to spend your appointment focused on you rather than moving between multiple clients.
Initial evaluations are generally 60 minutes, providing time for a detailed history, comprehensive movement assessment, treatment, education, and an initial plan. Follow-up appointments are also structured to allow meaningful one-on-one treatment and progression rather than simply repeating the same routine from visit to visit.
Your plan may change as you change. Early sessions may emphasize decreasing limitations in mobility or improving your ability to access a particular movement. As that improves, treatment can increasingly focus on coordination, strength, capacity, and returning to higher-level activities.
The objective is not dependence on physical therapy. It is giving your body the options and capacity it needs outside the clinic.
From feeling better to moving better
Pain relief matters. But for many of our clients, my back doesn't hurt today isn't the finish line.
You may want to lift without hesitation, play 18 holes without your back tightening on the back nine, return to running, travel without worrying about the flight or car ride, pick up your child, get back under a barbell, or work at your desk and still feel capable of exercising afterward.
Those goals help determine how we progress your care. Once your symptoms begin changing, we can gradually challenge your body with the movements, loads, and activities it needs to tolerate in real life.
Your treatment should prepare you for your life, not simply for the treatment table.

All of our sessions are 1on1 for 1 hour which give our therapists time to listen, thoroughly assess, and treat your problem.

Comfortable clothing that allows movement and breathing assessment. Preferably limit baggy clothing so assist in our ability to assess your posture/movement.

North Carolina is a full direct access state with no restrictions. This means you do not need a referral for physical therapy unless your insurance specifically requires one for reimbursement purposes.

We are an out of network provider for physical therapy services. We have adopted this model specifically because it gives us more time to assess and treat your specific problem without seeing multiple clients at one time.

Yes. Physical therapy can assess factors such as mobility, strength, movement strategies, and how your body manages the demands of work, exercise, or sport.

Not necessarily. Many people can begin physical therapy without having imaging first. Your physical therapist can recommend additional medical assessment when appropriate.

When appropriate, dry needling or therapeutic Pilates, or myofascial release may be incorporated as part of a larger treatment strategy. These techniques are tools we may use to help create an opportunity for better movement - and then reinforce that change through movement and strength.
