Looking beyond the painful area
Your neck doesn't function independently from the rest of your body. The cervical spine works closely with your upper back, rib cage, shoulders, shoulder blades, muscles, nervous system, and breathing mechanics. That matters because the place you feel symptoms isn't necessarily the only place contributing to them. For example:
Limited rotation through the upper back may require your neck to create additional movement when you turn.
Shoulder or shoulder-blade mechanics may increase the workload of muscles connecting the neck and upper body.
Other people have adequate mobility but lack the strength, endurance, or coordination required for their work or exercise demands.
Your therapist determines which factors are actually relevant to your symptoms rather than assuming that every possible contributor needs treatment.
Your first appointment at Congruency
Your initial physical therapy evaluation is typically about an hour and is spent one-on-one with your physical therapist.
We start with a conversation.
Your therapist will want to understand:
What you're feeling
When your symptoms began
What makes them better or worse
What treatments you've already tried
How your symptoms affect work or sleep
What activities you participate in
What you want to be able to do again
Then we'll assess your movement.
Depending on your presentation, this may include your cervical spine, thoracic spine, ribs, shoulders, shoulder blades, strength, muscular endurance, neural mobility, breathing mechanics, and functional movement.
If your neck only hurts when you're performing a specific exercise, bring that insight with you. Seeing the movement that creates the problem is essential.
What progress looks like
Progress isn't measured only by a pain score.
We also look at whether you're regaining the abilities that brought you to physical therapy in the first place.
That might mean:
Turning your head farther while driving
Finishing your workday more comfortably
Sleeping without repeatedly repositioning
Returning to lifting
Tolerating longer bike rides
Swinging a golf club comfortably
Exercising without repeatedly triggering symptoms
Feeling confident in your neck again

All of our sessions are 1on1 for 1 hour which give our therapists time to listen, thoroughly assess, and treat your problem.

Comfortable clothing that allows movement and breathing assessment. Preferably limit baggy clothing so assist in our ability to assess your posture/movement.

North Carolina is a full direct access state with no restrictions. This means you do not need a referral for physical therapy unless your insurance specifically requires one for reimbursement purposes.

We are an out of network provider for physical therapy services. We have adopted this model specifically because it gives us more time to assess and treat your specific problem without seeing multiple clients at one time.

Yes, recurring neck pain is a common reason people seek physical therapy. Rather than only addressing the painful area, an evaluation can identify relevant limitations in mobility, strength, endurance, shoulder mechanics, upper-back movement, or movement coordination that may be contributing to repeated symptoms.

Dry needling may be useful for some people when muscular pain or trigger points are contributing to neck symptoms. At Congruency, it isn't automatically used for neck pain or treated as a stand-alone solution. Your therapist determines whether dry needling is appropriate based on your evaluation and incorporates it into a broader rehabilitation plan when useful.

In many musculoskeletal neck-pain presentations, completely avoiding movement isn't necessary. The more useful question is which movements and loads your neck can currently tolerate and how those can be progressed appropriately. A physical therapist can help identify whether an exercise should be modified temporarily or whether another limitation needs to be addressed.
