What an appointment is like
Your first visit is a comprehensive one-on-one evaluation — never rushed, never shared with another client. We'll assess your posture, your shoulder and cervical range of motion, your ribcage mobility, and your shoulder girdle strength and stability, and talk through how your pain shows up in daily life, at work, or in your sport. From there, we build a plan specific to you.
Follow-up visits are also one-on-one with your treating physical therapist for the full session — combining hands-on manual therapy with active, guided exercise.

Initial evaluations run about 60 minutes; follow-up visits are typically 45–60 minutes, and you're always one-on-one with your treating physical therapist — never shared or handed off to an aide.

Congruency operates as a cash-pay, out-of-network practice. This allows us to spend full, uninterrupted sessions with you rather than dividing time to meet insurance productivity requirements. We're happy to provide documentation for out-of-network reimbursement if your plan allows it.

Comfortable clothing that allows movement and breathing assessment. Preferably limit baggy clothing so assist in our ability to assess your posture/movement.

North Carolina is a full direct access state with no restrictions. This means you do not need a referral for physical therapy unless your insurance specifically requires one for reimbursement purposes.

Most of the shoulder conditions we see — including impingement, tendinitis, and many rotator cuff and labral issues — respond well to physical therapy without surgery. If we do think a surgical consult is warranted, we'll make that referral directly.
